Twenty-One Years: The Plateau, the Collapse, the First Step Back

Medical Properties Trust paid a rising dividend for most of its life, reaching $0.29 a quarter by 2022. The chart above shows what followed: the 2023 cut to $0.15 and the 2024 cut to $0.08 as hospital tenants defaulted, then the late-2025 raise to $0.09. The year-by-year table shows income per share falling by nearly three-quarters in two years.

When the Tenant Is the Risk

A net lease transfers operating risk to the tenant — which works only while the tenant can pay. MPT's portfolio was concentrated in a few hospital operators, and when Steward Health Care and others failed, the rent that funded the dividend vanished with them. The company has since re-tenanted facilities, sold assets and refinanced, and the 2025 raise is the first evidence the rebuild is producing cash again.
